PHR shows a lot of sections, and at first glance it can feel like a lot to fill in. You don't need to. Enter the small set below and you'll already have a working health record — one you can show a new doctor, hand to a family member in an emergency, or use to keep track of what you're taking and why.

Patient is just you — one record. Every other section is a list you'll add to over time. Total Fields = Required × Items.

The app won't push you past the required fields. Notes, dates, providers, dosages, and the rest can all wait until you're ready to add them.

Three tips that make this easier

  • Start with Care Team. Once your providers are in, Conditions, Medications, and Immunizations all pull from that list — no retyping names.
  • Use NPI Lookup. On the provider form, the NPI Lookup button fills in most of the contact details automatically.
  • Add as life happens. New prescription? Add it. New diagnosis? Add it. The record builds itself once you're past the starter line.

Everything else — vitals, treatments, lab results, reports, legal documents, visits — is there when you're ready for it.
