Perplexity opens in Safari and doesn't require an account; Claude, OpenAI, Gemini, and xAI (Grok) are sent directly from the app using your own API key (each requires a paid subscription with that vendor — PHR doesn't provide AI service itself).

If your conditions are already in Apple Health, PHR brings them in for you — no retyping.
- Works straight from an Apple Health export — the .zip from Health → Export All Health Data on your iPhone or iPad; PHR unzips and reads it for you
- Pulls the conditions your doctors have sent to Apple Health — the Health Records you've linked under Settings → Health (not what's typed into your Medical ID)
- Pick the ordering provider once — Apple's export leaves it out — and PHR stamps it as the source on every condition
