Import from Patient Portals (CCDA)

Most patient portals — MyChart, Athenahealth, Cerner, eClinicalWorks, and others — let you download a Continuity of Care Document (CCDA). PHR reads that file and pulls your data into the right modules, so you don't have to retype anything.

Questions for Your Provider

Keep a running list of things you want to ask your doctor or pharmacist. Tap the question icon on any medication, condition, or lab result to start a new question linked to that record. Then print the list before your visit — with or without the answers you've collected — so the conversation stays focused.

Ask AI

Got a question about a medication, lab result, or condition in your record? Tap Ask AI on the Question screen and PHR sends your question to the AI service you've chosen. Perplexity opens in Safari and doesn't require an account; Claude, OpenAI, Gemini, and xAI (Grok) are sent directly from the app using your own API key (each requires a paid subscription with that vendor — PHR doesn't provide AI service itself).

  • Pick the AI service you prefer — or turn the feature off entirely
  • Attach a PDF, image, or text file with your question (Claude path)
  • Plain-English warning explains what's leaving the app the first time you use it, and again every time you attach a file
  • Saved answers are stamped with the model and timestamp so you know what produced them
  • Built-in disclaimer reminds you to confirm anything important with a licensed clinician

Health Check

Ask an AI to look at your record without giving up your privacy. Health Check builds a de-identified snapshot — age, sex, height, weight, plus whichever clinical categories you choose to include — and sends it to the AI service you've configured. The result comes back as a PDF attached to a Health Check record so you can save it, print it, or share it.

  • No name, date of birth, address, phone, email, or record numbers ever leave the device
  • Confirmation screen shows exactly what's about to be sent before you tap Send
  • Optional question field for things you specifically want addressed
  • Past Health Checks list keeps every run with its date, model, cost, and PDF for reference

Owner Tools — Backup, Transfer, and Audit

The Owner of a PHR device gets a set of system tools for keeping the data safe and portable.

  • Backup — Save a complete copy of every patient's database and all attachments to a folder you pick. Configure two backup locations and PHR writes to both whenever you back up.
  • Restore — Pull a backup back in. PHR compares table by table so you can see what's about to change before you commit, and one tap undoes the last restore if you change your mind.
  • Transfer Patient — Move a single patient's records from one iPad to another via AirDrop or the Files app. Useful when setting up on a new iPad or handing family record-keeping over.
  • Export to Excel — Hand off your records as an .xlsx file with one worksheet per section, frozen header rows, and matching column titles. Useful when sharing with a clinician who works in spreadsheets.
  • System Log — A running audit trail of important actions (logins, backups, imports, AI calls). Good for spotting if something unexpected happened.

  • Contact Support — Tap once to open a pre-filled email to support. The message is sent from your own email account, so no credentials leave the app.
  • Support Tickets — Every support email you send is auto-saved so you can refer back to what you asked.
  • LOINC Reference Table — Browse the bundled catalog of 678 lab test codes (across 25 panels) used to standardize lab result names. Search by name, code, or panel — handy when comparing reports from different labs.
  • Login Audit — A history of your own login activity for peace of mind.

If you've been tracking blood pressure, heart rate, or pulse oximetry on your iPhone or Apple Watch, PHR can pull that history straight in.
